Intermittent fasting produces weight loss primarily through calorie restriction, not metabolic rate increase. TREAT trial (Lowe 2020, NEJM Evidence, n=116): 16/8 IF produced no greater weight loss than unrestricted eating at 12 months — both groups lost similar amounts. 5:2 (Harvie 2011): equivalent to continuous restriction at similar calorie deficit. IF is a tool for creating a deficit, not a metabolic override. Who should not fast: people with eating disorder history, insulin-dependent diabetes, pregnancy, under-18s.
