Weight loss is one of the most effective non-pharmacological interventions for hypertension. Magnus et al. (1997, meta-analysis): approximately 1 mmHg systolic and 1 mmHg diastolic reduction per kilogram lost. DASH trial (Sacks 2001): diet + weight loss reduced systolic BP by 11.4 mmHg. Mechanisms: reduced cardiac output from lower blood volume, improved arterial compliance, reduced sympathetic nervous activity, reduced renin-angiotensin system activation. NICE recommends weight loss as first-line for overweight/obese hypertension.
